In today, "public welfare" has become a time to discuss the health of the population in a modern concept, but for the most precise meaning is unclear. Moreover it as opposed to public hospital restructuring and introduction of market competition mechanism, the shield that medical and health cause of public welfare can only be done by public hospitals, private hospitals and market mechanisms are unable to guarantee public welfare. Therefore, public welfare become the primary prerequisite to understand correctly.
 
    To understand the connotation of public welfare, must first clear the principle of "superior". The basic goal of medical and health cause lies not in the public welfare, but in the upper goal "health promotion". For that matter, any form of system of ownership of medical institutions have public welfare, it also means that the public cause of medical and health is not necessarily by public hospitals to reflect and assurance. Fail to improve if the government has increased financial input in medical and health service performance, only to solve the problem of "expensive" but not solve the problem of "difficult", just reduce the patients, the proportion of out-of-pocket without control of the excessive growth of total medical expenses, still cannot be guaranteed as implementation or the public welfare.
 
    Therefore, to ensure the public welfare is not only to increase government financial input in medical, more important is to enhance the overall performance of the medical service system. As is known to all, public hospitals due to its nature of the institution and its flesh and blood contact closely with the health administrative department, especially in the public hospital monopoly pattern, cannot fundamentally overcome due to lack of external competition and efficiency of its produced by cost control consciousness, resource waste and so on. Must construct the system of fair competition environment, formed between public hospitals and private hospitals, orderly competition between public hospitals and private hospitals, through the competition mechanism to improve the performance of the whole medical and health services to provide system. In general, civilian battalion hospital and public hospitals have the endogenous nature of public welfare, and derived the realization of the public welfare depends on both the fair competition.
